EMMC has joined together with Penobscot Community Health Care (PCHC) and St. Joseph Healthcare to promote medication safety in the greater Bangor region. The goal of this collaborative effort is to reduce medication errors and promote communication between consumers and their caregivers. We want to encourage consumers to take an active role in their medication safety by maintaining a record of their personal medical information.
A free document named "Your Guide to Medication Safety" is now available to provide consumers with a simple card to record all medications – prescriptions, herbal supplements, over-the-counter medications and vitamins, as well as immunizations, allergy and primary care provider information. Everyone is encouraged to carry this medical information record with them at all times so it can be presented to caregivers at all points of care: doctor's offices, emergency rooms, and pharmacies.
The card is available at Eastern Maine Medical Center, Penobscot Community Health Care, St. Joseph Heathcare, and at various local pharmacies.
